| I/we hereby apply for membership of Vere e.V, the association for the
Return an Environmentally Sound Disposal of Electrical and Electronic Equipment. |

The annually membership fee is 30,00 € and will be raised with an invoice at the beginning of the year.

Through becoming a member of the VERE e.V. disposal association there is no agreement of future disposal means over the VERE disposal association.

| Herewith we declare our participation from the date of signing to the direct debiting of VERE e.V. and authorize the VERE e.V. revocable to debit from our account the annual amount of 30,00 € at the beginning of every year by means of direct debiting. |
